Please note that when you fill out your form to request midwifery care, you will be re-directed to a secure site which is HIPPA compliant and protects your personal information. You will be contacted within 2-4 business days after completing the form.

If you do not have a health care number yet, please fill out the form and our admin will discuss your options for care with you before your appointment is booked.
